Okay, folks, who else felt like all the delicious promised h/c got UNFAIRLY RIPPED AWAY last night? Yeah. What is this "three weeks later" nonsense?

Where was the head trauma? The hobbling around on crutches? The possible coma? Bobby having to take care of unconscious!Sam and loopy-on-painkillers!Dean?

And THEN there was all the during-and-post episode stuff that could have happened! Like Dean having a "mishap" cutting off his own cast. Sam dealing with Lucifer. ALL THE H/C!

In short, there needs to be comment-fic to fill in these terrible, terrible lapses in judgement on the writers' part, am I right? Of course I am!

This got a little twisted in my mind as I wrote it - so hopefully it's okay. I wouldn't be offended if anyone else wanted to take a stab at it.

“Hi Sam,” Lucifer perched on at the foot of Sam’s bed. His legs crossed and his fingers steepled beneath his chin. Sam pressed his thumb to the fragile skin of his palm. He could still feel the healing wound pulsing before he reverted to the trick Dean had taught him. Now it felt like bolts of fire were spreading out across his palm from how desperate he was getting to get rid of Lucifer.

It didn’t work though. Lucifer was still there. With inhuman grace he stood from the foot of the bed and moved towards the machines that were beeping and clicking in effort to ensure Sam’s life. Lucifer let his fingers dance along the IV line before tapping Sam’s arm lightly, “This is all so silly isn’t it?”

“You’re not real,” Sam swallowed, his eyes aching and his head hurt so badly he felt like he was going to vomit. “You’re not here.”

“But am I?” Lucifer clucked his tongue. Echoes of Sam’s name bounced off the stark white walls. Sam rubbed at his eyes and tried to focus on the wavering form of his worst nightmare. “I think we both know that I am. I think that we both know that the illusion your mind has built up in order to protect yourself from me is finally crumbling and you can’t do anything to stop it. Not anymore.”

“I’m in a hospital,” Sam croaked out. “I’m in a hospital and my brother is in that bed next to me.”

“Yeah?” Lucifer tilted his head almost as if he were challenging Sam. He backed up to the partition and flung it back. The bed was empty, sheets tightly folded along the mattress.

“Dean,” Sam whispered out.

“You ever wonder why you’re in a hospital of all places?” Lucifer flopped back on the empty bed with his hands folded over his chest. “I mean the thought never crossed your mind?”

“You’re not real.”

“Sam,” Lucifer drawled the name out before he smiled, “Sammy.”

“Don’t!” Sam flung an unsteady arm out, “You have no right.”

“Being the only resident in your grapefruit allows me to call you whatever I’d like.”

“I’m in a hospital,” Sam began to recite the mantra over and over in his head as he pressed his thumb even harder into his wounded palm. “This is not real.”

“Oh, back to the subject at hand. Why you’re brain decided that a hospital was the ideal place for your protection. I mean I’m finally breaking you down and I’m finally understanding this,” Lucifer vaguely gestured around the room, “Every bad thing, well what you consider to be the worst things, have happened to you in a hospital.”

“Shut up!”

“Your brother almost died in here, remember? First it was his heart – but you fixed that, didn’t you Sammy? Reapers I believe. Then there was the car accident. Oh I remember that one. It was quite the even outside of the cage. All Winchesters hanging on for life. All expect little Sammy. Dean was stuck in the in between for a bit. His soul just waiting to be stolen but he took care of that soon enough.”

“You’re not real,” Sam scratched at the IV in his arm and grimaced as he pulled it free.

Lucifer leaned close to Sam’s ear, his eyes angled towards the empty bed next to Sam’s, “Then how I did I do that?”

Sam looked across the room to where Lucifer was looking. His heart pounded in his chest as he watched Dean curl onto his side and his arms tighten around the pillow he was resting on. Almost as if he had been there the entire time. Sam’s eyes darted around the room searching out the devil himself but finding no one. His head was pounding heard enough he felt the need to vomit. He couldn’t focus on anything expect the blurry image of his brother – softly breathing in the bed next to him.

Oh this is a lovely idea rather :D

Bobby is out on a supply run. Dean can't move with his leg in a cast and Sam while unconscious has a seizure/hell nightmare. Having a broken leg doesn't stop Dean from getting to Sam's bedside. Lots of comfort of course!

I want Bobby getting them to the cabin and having to deal with in-pain-and-not-mobile Dean, and mostly-unconscious Sam. Maybe when Sam is conscious, he's having more hallucinations than seeing reality while his brain recovers. I don't know...I just want some Bobby mother-hen.

Alright lets do this thing ^^

We saw Dean and Bobby getting into those weird Soap operas and Sam being clearly uninterested.

But what if Sam wasn't allowed to read at first and had to watch the horrible Soaps (because outvoted!.... and he had to watch instead of sleeping because someone had to translate or something?^^)

...shut up I need me some schmoop and he could still have like terrible headaches made worse by the spanish and not tell anyone or Dean would force him to take his mind of his broken bone or somthing so it totally counts as h/c XD

This is for any Sam girls who were a little disappointed by the lack of a hospital bed scene for Sam.

Prompt AU: Dean wakes up from the morphine to find his leg in a cast and Sam hooked up to different machines in the bed next to him. It sucks royally to be a Winchester nowadays

I was going to run my errands and then come back and prompt, but... Well. YES. *cracks knuckles*

One of the complications of a long bone fracture is a fat embolism. Basically, bone snaps and, for ease of prompting, the contents are released into the blood stream where they can travel to the lungs, heart, etc. This is BAD. Dean, who makes his escape from the hospital w/o proper medical care is fine for the first day and develops a rash on his upper body, his heart rate sky rockets, he starts breathing rapidly, and well look here: http://anestit.unipa.it/gta/fat_embolism.html (see clinical presentation). How Sam who is dealing with a concusion on top of an already scrambled brain manages to get him medical attention by himself or with Bobby's help is up to you.

The drive out from Sioux Falls is a bit worse than Dean bargained on. I mean really, do you have any idea how much a compound fracture HURTS? And moving it around/having it hang down like that, just after it happened? And the long crutches-walk out of his room burned up just about all the morphine.

Once the swelling starts to make the cast tighter, the pain is almost beyond description.

Speaking of broken bones... when you've broken a bone and are lying around not so active, the blood can pool in the lower extremities and form blood clots. In a hospital you'd combat this with sequential compression devices (the leg squeezie things, if you've seen them) and low molecular weight Heparin injections in the belly. Dean's obviously gotten some kind of medical care after their flight from the hospital be it legally or through some of their less than savory connections. So, Bobby's stolen Dean an SCD machine and sleeve for his good leg and every day Sam has to give Dean a shot in the belly. You can make this schmoopy or not as you so desire. (Not that it really matters, but Lovenox (the shot in question) is a big pain to get at home because of insurance coverage due to the expense. So, one assumes that Bobby again came by this med through less than legal means.)

Side tangent idea to this, for the Sam girls:

I was doing research into traumatic brain injuries, and there's at least a 15% increased risk in blood clots there too, mostly due to inactivity, but also increase in inflammatory mediators. (Not to mention the blood clot risk in the brain area due to injury and inflammation around the site of impact, but that's another story.) Wouldn't it be ironic if, after dutifully helping Dean with his shots, Sam ends up with a blood clot because he's too dizzy and lethargic and out of it from headaches and spacing out with hallucination stuff to move around enough? Then they could both be on Lovenox, hehe.

The Winchesters are bad enough on their good days but when they're hurt? They're 10 times worse...especially when they are stuck on the road and on the run.

Bobby's POV driving to Montana with Dean's leg cast and Sam awake but seeing things...

After a few days on the road with the boys, Bobby has a new appreciation of what John had to deal with when the boys were growing up.

Sam doesn't wake from the concussion with all of his faculties intact. His speech is slurred and he has difficulties getting the correct words out. Like, maybe he wants to say "hey Dean, where's Bobby?" and what comes out instead is "Dean rabbits run darkly?" Does the brain damage heal? How does Dean manage to keep Sam calm and still communicate? How's Bobby manage this along with Dean in pain and having difficulty moving because the cabin isn't exactly handicap friendly?

Someone shakes Sam's shoulder to wake him. A doctor with a pointy nose and huge horse teeth who Sam vaguely remembers from forever ago when they dragged Dean off on a gurney. The word "Powers" is embroidered on her shirt, and he squints at it for a second before he understands it as her name.

"Sam? Your brother's out of surgery," she says, giving him a well-rehearsed half-smile.

He peels himself off his plastic chair and nods. He feels like he's been stowed away in a crate to cross the Atlantic on a steam ship. But he figures Dean feels a hell of a lot worse.

"How's he doin'?" he asks, following the doctor down the hall.

"He should wake up soon. We had our work cut out for us with his tibia, I'm afraid. Since it hadn't healed properly, as soon as he exerted pressure on it, the bone splintered even further."

Sam winces and nods. This doesn't sound too good.

"When he regains consciousness," she starts, pausing outside a door marked ORTHO-ICU RM 204, and Sam suddenly can't stop staring at it. "Sam?"

His head twitches and he tears his eyes away, looks at the troubled face of Doctor Powers. "Sorry. Yeah."

She starts again slowly, as if she's helping him learn English. "When he regains consciousness, he might be upset by what he sees. If you can stay calm and attentive, that would really help. Okay?" she asks, nodding towards her hand on the door handle, waiting for some sign that Sam is ready to face whatever horror show is on the other side of that door.

He isn't too sure that he is, but he takes a deep, slightly unsteady breath and nods anyways.

There's only one bed in the cramped and windowless room. Dean's lying still on it, his leg hanging from a metal frame that arches over the bottom half of his bed. The frame is rigged to wires that are fastened to rings that are clamped to pins and steel bolts and more wires and, Christ, somewhere in the center of this complex industrial matrix, is Dean's leg. It looks like a goddamn modern-day torture device.

It's a lot to process. "What..."

"Intense, I know. But it's your brother's best chance at a full recovery."

"Okay. Jesus. How long does this stuff..." Sam tapers off again, waving his hand at the web of surgical steel.

"We'll keep him here four to six weeks to be safe, since there's a risk of infection. Then he'll need around eight to ten more weeks in the external fixation. If no other surgery's necessary after that, we'll fit him with a leg-brace and start some serious physio."

"That's like four months in that thing. God."

"Sam, most patients don't recover from an injury like this for a good year. If they fully recover at all."

"Okay. But it's just... It's just a broken leg. What happened?"

"Hey, I know this seems crazy right now, yeah? But these things do happen. The other day I operated on a woman who tripped over a garden hose. She tore so much cartilage in her wrist she'll have to learn to write with her left hand. I'm really sorry. It's not easy to see a healthy young man like your brother pull such a short straw."

"Yeah, well. You don't know the half of it," Sam says, slowly edging closer to the bed and trying his best to process the visual information without curling into the fetal position.

Did you know that the tibia is the bone most prone to nonunion after fracture? Nonunion meaning that it doesn't fuse back together/heal. One assumes especially if one has fled the hospital to avoid being eaten by the staff. This means that the leg can be fucked up for life, nerve damage, pain, foot drop... Poor Dean.

The yellow ones are as thick as bullets, almost wedging in his throat before Dean tosses back a quick ounce of whiskey to wash them down. He wipes his mouth off on his sleeve, leans his head back against the couch and lets out a long, tight breath.

The clock’s ticking while he waits for the constant barbed wire of pain in his leg to dim around the edges. Whatever this shit is, it works slow. Dean straightens up, cracks his neck, and the time on the clock reads 1:13 AM. Half an hour.

Sam’s been down since ten or so. He insisted that he was fine, and Dean knows he’s got no right to force the truth out of his brother when he’s been lying to his face right back. Bobby’s somewhere he can’t remember.

He’s been laid out on this couch with a pint of Jack and the cast propped up on a stack of pillows to keep it elevated, sunk in a mire of alternating self-pity and disgust. Just another Saturday night. The whiskey’s helping, though, like it always does.

After a few more minutes of numbness, Dean sits up, his stomach suddenly gnawing. There’s half a leftover pastrami Reuben in the fridge and Dean wants it as much as he's ever wanted anything. It’s all he can think about right now for some reason - coleslaw and Russian dressing on rye.

He props himself up on the couch arm and comes to a shaky stand. This cabin is so pitch dark at night it's almost un-fucking-inhabitable, all dust and loose floorboards and cobwebbed corners for brown recluses to hide in and wait until he falls asleep to crawl all over his goddamn face. The thought gives Dean a horrible itch right under his nose. He rubs it with his free hand, rough, and then his center of balance is shifting and his good leg decides to buckle under his weight. When his knees hit the floor, the impact sends up a cloud of dust thick enough to make him sneeze twice in a row.

“Fuckin' A," he moans, sniffling.

Dean roots around in the darkness for his cane and gives it another shot, choosing his steps carefully. The floor’s kind of wobbling in front of him, blurring in and out of focus, and he has to stop for a moment to catch his breath in the kitchen doorway. Jesus, his leg is itching.

Rufus's sorry excuse for a fridge doesn't even have a working light. Dean digs around on the top shelf blindly until his fingers brush the paper-wrapped sandwich.

He sits down heavily on the floor, the fridge door swinging shut next to him. There’s a little ambient light coming in from the side yard and he squints, trying to unwrap the Reuben. His head feels heavy, thick, like it's been packed with cotton, and his chin’s starting to droop. Fuck that. He’s not sleeping, not yet. Someone’s got to stay up and make sure Sam’s all right, be there if he needs help, keep him safe, protect the house and everything. He’s just got to keep it together a little longer.

With bleary eyes, he pulls the whiskey flask out of his coat pocket and tries to unscrew the cap, but his stupid fingers won't do what they're supposed to and a bunch of it spills down all over his chest, stinging and cold.

"Fuck," Dean mumbles, pissed at himself. He takes a long, angry pull from the bottle, then follows it with a sloppy bite of the sandwich. It feels like there are lead weights on his eyelids trying to drag them shut. He’s not going to pass out. Not in the kitchen. That’ll earn him a lecture from Bobby and he hates hearing them because he knows Bobby’s right. Or, even worse, one of those looks from Sam that just eviscerates him, withers him on the spot, makes him feel small and pathetic and guilty, guilty, guilty.

Vertigo sweeps over him in a sickening wave, blotting out his vision, and Dean realizes he’s got to make it back to the couch, now, fast fast fast. He slumps forward, lands on his elbows and tries to crawl but everything is moving too slow, all warped and dizzy and screwed up. The weirdest part is that his leg feels fantastic.

Dean can hear his voice trying to say something before the walls crash down and his ears are ringing and his stomach is in free fall and there's copper in his mouth and the floor rushes up to meet him --
